Massage Therapy Invoice Template: Receipts, HSA and Packages
Use this massage therapy invoice template to bill sessions, packages and add-ons, and give clients receipts that work for HSA or FSA claims. Sample included.
Quick answer: A massage therapy invoice shows your business name, licence number and credentials, the client's name, the session date, the type and length of massage, the fee, add-ons, package credits or discounts, the amount paid and the method. For HSA or FSA clients, use clinical wording, list the treatment dates, and keep tips on a separate line.
Most massage clients pay at the front desk and leave, so billing seems simple. The trouble starts later: a client asks for a receipt for their health savings account, a corporate client needs an invoice for chair massage, or someone disputes how many package sessions they have left. A clear massage therapy invoice, built in a free invoice generator, solves all three with the same template.
This guide covers what to put on a massage invoice or receipt, how HSA and FSA reimbursement works, pricing models and typical 2026 rates, packages and gift certificates, a worked example with real numbers, and the mistakes that cost therapists money and goodwill.
What a massage therapy invoice should include
Whether you call it an LMT invoice, a massage receipt or a bill, the essentials are the same:
- Your business details: practice name, address, phone and email, plus your name and credentials, such as Jane Lee, LMT.
- Licence number: your state massage licence, and your NPI if you have one.
- Client name: the person who received the massage, which matters for HSA claims.
- Invoice number and date: a sequence such as MT-2026-0318. Our invoice number generator helps you pick a format.
- Service lines: date, modality and duration, such as Deep tissue massage, 60 minutes.
- Add-ons: hot stones, cupping, aromatherapy or extended time, each on its own line.
- Package or membership credits: the session used and sessions remaining.
- Payment: amount paid, method and balance due, ideally zero.
Keep clinical notes, such as SOAP notes, off the invoice. They belong in the client record. The invoice only needs enough detail for the client, or their plan administrator, to see what was provided and paid.
Mobile and in-home massage invoices
Mobile therapists carry extra costs that a studio does not: travel time, fuel, parking and the wear on a portable table. Show these honestly rather than hiding them in a higher session price. A travel fee line, such as Travel fee, 12 miles each way, or a flat zone charge for addresses outside your usual area, tells the client exactly what they are paying for. Add parking or toll charges at cost with the receipt. Put the service address on the invoice as well as the billing address, because hotel guests, event clients and families booking for a relative often pay from somewhere else. If a client is not home or the space is unsafe to work in, your cancellation policy should say whether the travel fee still applies, and the invoice should note it.
HSA and FSA massage receipts: what clients need
This is the most common reason a massage client asks for a formal invoice. The rules come from IRS Publication 502, which defines medical expenses as the costs of diagnosis, cure, mitigation, treatment or prevention of disease, and excludes expenses that are merely beneficial to general health. Massage is not on the publication's list of named treatments, so it only counts when it treats a specific condition.
In practice, HSA and FSA administrators usually ask for two documents:
- A letter of medical necessity from the client's physician or other licensed provider, naming the diagnosis, explaining why massage treats it and stating the recommended frequency.
- An itemised receipt from you, showing the patient's name, each date of service, the service, your credentials and the amount paid.
Your part is the receipt. Use clinical wording such as Therapeutic massage, 60 minutes, lower back, rather than Relaxation spa package. Do not add diagnoses you did not make, and do not word a relaxation visit as treatment. You can read the full IRS Publication 502 on medical expenses, and plan rules can be stricter than the IRS standard.
When a superbill is a better fit
Some clients want to claim massage from health insurance rather than an HSA, usually when massage is part of a physical therapy or chiropractic plan. Insurers expect diagnosis and procedure codes on a superbill rather than a simple receipt. Our sibling guide to the superbill template explains the fields, codes and process.
Massage pricing models and typical 2026 rates
Published 2026 price guides put a 60-minute massage at roughly $60 to $150 in the US, with many markets landing around $80 to $120 before tip. A practitioner survey by a massage professional association reported an average of about $87 for a 60-minute session. City centre and mobile therapists usually charge more, and specialised work such as sports, prenatal or medical massage often carries a premium. Treat these as rough illustrations, not a price list.
| Model | How it works | Invoice line example |
|---|---|---|
| Per session | Fixed price by length, such as 30, 60, 90 minutes | Swedish massage, 60 min |
| Package | Prepaid bundle of sessions at a discount | 5 x 60 min package, 10% off |
| Membership | Monthly fee includes one session, extra sessions at a member rate | October membership, includes 1 x 60 min |
| Mobile or in-home | Session fee plus travel premium | Mobile session, 90 min, plus travel fee |
| Corporate or event | Hourly or per-day rate for chair massage | Chair massage, 4 hrs, 2 therapists |
If you are setting or reviewing your prices, our guides on setting your rates and raising prices walk through the maths of costs, hours and demand.
Packages, memberships and gift certificates
Packages and memberships improve cash flow, but they create a liability: you have been paid for work you have not done yet. Your invoices need to make that obvious.
- At purchase: invoice the package with the number of sessions, the full price, the discount and the total paid.
- At each visit: issue a receipt showing Session 2 of 5 used, 3 remaining, with zero to pay.
- Expiry and refunds: state your policy in writing. Many states regulate how gift certificates and prepaid services can expire, so check before you set dates.
- Gift certificates: invoice the buyer, then treat the redemption as a zero-balance session for the recipient.
Memberships are billed the same day each month. Our guide to recurring invoices covers how to keep monthly billing tidy, and the deposit invoice guide applies to large event bookings where you take money up front.
Sample massage therapy invoice with real numbers
Here is an illustrative example. A licensed massage therapist sells a client a package of five 60-minute therapeutic massages at $95 each, with a 10% package discount, and the client also books a single 30-minute hot stone add-on at $20 on the first visit.
| Description | Qty | Rate | Amount |
|---|---|---|---|
| Therapeutic massage, 60 min, prepaid package | 5 | $95.00 | $475.00 |
| Package discount, 10% | 1 | -$47.50 | -$47.50 |
| Hot stone add-on, 7 Oct 2026 | 1 | $20.00 | $20.00 |
| Total | $447.50 | ||
The arithmetic is 5 x $95 = $475, less 10% ($47.50) = $427.50, plus the $20 add-on = $447.50. Each session in the package is effectively worth $427.50 / 5 = $85.50, which is the figure to show on any HSA receipt for a single visit. If the client tips $15 by card on the day, record it as a separate tip line so the HSA receipt still shows only the treatment.
How to build it in the generator
- Open the USA invoice generator, or the UK preset for British practices.
- Add your logo, practice name, address, credentials and licence number.
- Enter the client's name, the invoice number and the date.
- Add the package line, a discount line and any add-ons. Our guide to showing a discount on an invoice explains the options.
- Leave tax at zero unless your state taxes massage or you are selling taxable products.
- Enter the amount paid so the balance due shows zero, and note the payment method.
- In the notes, add the package terms, such as 5 sessions, valid 12 months, 1 used.
- Download the PDF and email it, or print it at reception.

Tips, cancellations and no-shows
Three small policies prevent most disputes in a massage practice:
- Tips: record them separately from the service fee, so receipts are accurate and your income records are complete. Our article on the no tax on tips rules for the self-employed explains the new federal deduction and who it covers.
- Late cancellations: many practices charge 50% to 100% of the session for cancellations within 24 hours. Put the rule on your booking page and invoice it as Late cancellation fee, 14 Oct.
- No-shows: usually the full session fee, or one session deducted from a package.
Fees for missed appointments are not medical expenses, so keep them off any HSA receipt. Our guide to late payment fees covers the wider rules on charging extra.
Sales tax and VAT on massage
Massage therapy is not subject to sales tax in most US states, but a handful of states and some cities tax certain personal care or spa services, and products you sell, such as oils, balms or gift sets, are often taxable as goods. If you sell products and services together, consider separate invoices, because our generator supports one tax rate per invoice. The sales tax calculator and our upcoming guide on how to calculate sales tax help when tax applies. In the UK, VAT only applies once you are VAT-registered. This is general information, not tax advice.
Corporate chair massage and event invoices
Corporate wellness days, conferences and sports events are invoiced differently from walk-in clients. The client is a business, the work is booked in advance, and payment usually follows on terms such as net 15 or net 30. Include:
- The event name, date and location.
- Number of therapists and hours each, such as 2 therapists x 4 hours.
- Travel, parking and equipment fees.
- A purchase order number if the client provides one.
- Payment terms and the due date. Our invoice payment terms guide explains the options.
Taking a deposit for event bookings protects you from last-minute cancellations. The personal trainer invoice template guide covers similar session and package billing if you also offer fitness services.
Common massage invoice mistakes
- Spa wording on medical receipts. Relaxation package language makes HSA claims harder.
- Missing licence number. Administrators and insurers often need proof of licensing.
- Tips mixed into the fee. It overstates the medical expense and muddles your income records.
- No package tracking. Show sessions used and remaining on every receipt.
- Unclear cancellation policy. Charging a fee nobody agreed to leads to refunds and bad reviews.
Create your massage therapy invoice
A massage therapy invoice should be quick to issue and easy for the client to use, whether they are paying from an HSA, buying a package or approving a corporate event. Show your credentials, describe each session clearly, track package credits and keep tips separate. Build yours in the free invoice generator and download a PDF receipt in a couple of minutes.
Frequently asked questions
What should a massage therapy invoice include?
Include your business name, your licence number and credentials such as LMT, the client's name, an invoice number, the session date, the type of massage and its length, the fee, any add-ons, discounts or package credits, tax if it applies, the amount paid and the payment method. For clients claiming from an HSA or FSA, add your NPI if you have one and keep the wording clinical rather than spa-like.
Can I pay for a massage with my HSA or FSA?
Sometimes. IRS Publication 502 defines medical care as costs of diagnosis, cure, mitigation, treatment or prevention of disease, and excludes expenses that are merely beneficial to general health. Massage for relaxation does not qualify. Massage to treat a specific diagnosed condition can qualify, and most plan administrators ask for a letter of medical necessity from a doctor plus an itemised receipt from the therapist.
What is a letter of medical necessity for massage?
It is a short document from a physician or other licensed provider that names the patient's diagnosis, explains why massage therapy treats it, and states the recommended frequency and length of treatment. HSA and FSA administrators use it to decide whether massage is a medical expense. The therapist does not write it, but your receipt should match it: same patient, condition-related service, dates and amounts.
How much does a 60-minute massage cost in 2026?
Published 2026 price guides put a 60-minute massage at roughly $60 to $150 in the US, with many markets around $80 to $120 before tip. A practitioner survey by a massage professional association reported an average of about $87 for a one-hour session. Big cities, mobile visits and specialised work such as sports or prenatal massage usually cost more.
Should tips go on a massage invoice?
Record tips separately from the service fee. If a client adds a tip by card, show it as its own line after the session total so the receipt is accurate. Tips are not usually reimbursable from an HSA or FSA, so keep them off any receipt the client will submit, or clearly separated. Tips are income for the therapist and need to be recorded for tax.
How do I invoice a massage package?
Invoice the package when it is bought, showing the number of sessions, the price per session, any package discount and the total. Then, at each visit, give a receipt that shows the session used and the number of sessions remaining. State your expiry and refund policy for unused sessions in writing, and check your state's rules on prepaid services and gift certificates.
Is massage therapy subject to sales tax?
In most US states massage therapy is not subject to sales tax, but a few states and some localities tax certain personal or spa services, and products you sell, such as oils or lotions, are often taxable. Rules change, so check your state revenue department. In the UK, VAT only applies once you are VAT-registered. This is general information, not tax advice.
What is the difference between a massage receipt and an invoice?
An invoice asks for payment and shows what is owed. A receipt confirms that payment was made. Many massage therapists take payment at the end of the session, so the document they give is really a paid invoice or receipt, showing a zero balance. Corporate chair massage or event work is usually invoiced in advance or afterwards with payment terms.
Do massage therapists need to include a licence number?
Many states require licensed massage therapists to display their licence, and some require the number in advertising. Putting it on invoices and receipts is good practice, and HSA or insurance reimbursement often depends on showing the therapist is licensed. Check your state massage board for the exact rules on where the licence number must appear.
